Warning: Participation in the Warri-Effurun Peace Marathon can be a serious threat to the health of individual who are not in excellent physical condition. For and the consideration of my participation in the Warri-Effurun Peace Marathon, I myself, my Executors, Administrators, heirs and assigners hereby release and discharge the Athletics Federation of Nigeria (AFN), Tamatare SPA & GYM Management, the organizing committee and all sponsors, partners, agencies, subsidiaries, affiliates and beneficiaries jointly and severally hold and waive harmless from and against any actions, claims, injuries, incapacitation / death demands, liabilities, loss, damage or expenses of whatever kind and nature including, but not limited to, attorney fees which at anytime may be incurred by reason of my participation in or my preparation for any of the said event.

I attest and verify that I have full knowledge of the risks involved in this event and I am physically fit and sufficiently trained to participate.

The undersigned grants full permission to any and all forgoing use to his or her likeness, including photographs and video tapes for publicity and advertising purpose without compensation.

Participants should submit a doctor’s fitness report
